Revision Rhinoplasty
This patient is a 23-year-old female with a history of two previous rhinoplasties, the last one approximately two years ago, where Gore-Tex was placed onto her nasal dorsum. Since that time, she has had a tender and increasingly swollen middle third and nasal tip with a large poly-beak abnormality. She additionally has significant difficulty breathing through her nose.
On physical examination, she was found to have a granuloma in the nasal valve area on the right as well as significant nasal valve collapse and an enormous poly-beak deformity. Her inferior septum had no cartilage present and she also had severe tip ptosis with collapse of the external valves. She additionally had significant alar and tip asymmetries. These deformities are notable upon review of her before photos from both the lateral and frontal views.
The patient underwent surgical reconstruction of her nose with rib cartilage grafting.
The patient is very pleased with her overall outcome. Not only were her extensive cosmetic concerns addressed but the patient has had noticeable improvement in her breathing ability. Her frontal photos indicate the overall appearance of the nose is straighter. The size of the nose is now much more proportionate to the other features of her face. The distracting poly-beak deformity has been modified in a manner that allows for a very natural look. This modification is particularly evident in the comparison of the lateral before and after photos. The profile is now pleasing to the eye as the severely abnormal hump has diminished.
