Lower Facelift, Full Face Fat Transfer, and Chin Augmentation
This patient is a 60-year-old female who is concerned about the appearance of her lower face and lower eyelids. On physical examination, she has significant jowling with excess jowl tissue. Additionally perceptible in the lateral before photo is platysmal banding and drooping of the neck and skin. She also has an undersized chin. Her lower eyelids and cheeks show a mild orbital hollowing and cheek volume loss. Noticeable upon review of the patient’s oblique before photo is significant superior orbital rim volume loss and perioral volume loss with acne scarring around her mouth. To address her concerns, I recommended a lower facelift with jowl microliposuction, neck liposuction, a platysmaplasty and medium extended anatomical chin implant. I also recommended full face autologous fat transfer. The patient elected to proceed with these procedures.
The patient is very pleased with the results she desired of the more youthful appearance. Immediately noted in her frontal after photo are her vibrant eyes highlighted by the rejuvenation of her lower eyelids by fat transfer. The lower eyelids no longer drag down the appearance of the face. The continuity of lifting continues from the midportion of the face to the lower face and neck due to the full facelift. The neck line has been brought to a natural contour that allows the smooth jawline to stand out and no longer distracts the eye. The lateral after photo not only highlights these improvements but the symmetry to the portions of each third of the face is an outstanding change due to the chin augmentation. The chin is no longer being lost in the neck line and really provides the patient with a beautiful confident appearance.
