This patient presented after facial trauma resulting in a nasal fracture many years prior. This resulted in difficulty breathing through the nose. She also had cosmetic concerns regarding a dorsal hump and the overall size of the nose. Her preoperative exam revealed a deviated septum with collapse of the nostrils during breathing. Her preoperative lateral and ¾ view photos demonstrate a dorsal hump, as well as an over-projected and under-rotated tip. Her frontal view shows a bulbous nasal tip. The patient elected to undergo a functional septoplasty with spreader graft placement and a cosmetic rhinoplasty to address these concerns. Her postoperative lateral and ¾ views show a straightened nasal dorsum with improvement in projection and tip rotation. Her frontal view shows straightening of the nose as well as improvement in her nasal tip. Functionally, the patient reported improvement in her nasal breathing and she no longer has collapse of the nostrils with breathing. She is very happy with her results!
