Rhinoplasty
This patient is a 21 year old female with severe nasal obstruction due to septal deviation, nasal bone deviation and collapse of her upper lateral cartilage with internal valve obstruction. Her preoperative frontal view demonstrates how the nasal bones are deviated to the right with a collapse of the left upper lateral cartilage. The tip then curves back to the left. Her base view shows the nostril asymmetry and presentation of the caudal septum into the right nostril. She did not present primarily for cosmetic changes, but did want the nose straightened. She was open to other cosmetic improvements however.
She underwent an open septorhinoplasty. Spreader grafts were placed on the left in a layered fashion, the caudal septum was repositioned and osteotomies were performed. No shaving of the dorsum was performed but the tip was narrowed and projected slightly.
Her post-operative frontal view shows how the nose is now straight with both aesthetically and functionally appropriate width. The tip has been subtly refined. The base view shows how the columella is now in the midline and the nostrils are symmetric. The lateral view shows how by increasing the projection slightly the dorsal line is now slightly convex and more pleasing.
