Sculptra-full face volume enhancement
This patient is a 43-year-old female with a history of consistent use Botox® as well as perioral filler. She presents in clinic concerned with the appearance of her periocular rhytids, the small hollow at the corner of her mouth area and general facial aging concerns. Upon examination, her lower eyelids have very fine rhytids and should only be treated with skin care and Botox at this point. The main feature notable in her facial aging is that she has very deficient cheek projection of the upper cheek. This is evident in her lateral before photos. We discussed this finding and that she would benefit greatly by volume enhancement of her cheeks. A variety of treatments were discussed which would include autogolous fat grafting, filler, or implant. In addition, we had a discussion regarding filler and the longevity of Sculptra versus Restylane/Juvederm that require repeat treatment to maintain the desired benefit. Of these options discussed she can really not afford the down time of surgery and so she elected to proceed with Sculptra.
Her results are subtle, as her facial changes look very natural. Take a close look at the far cheek in her oblique photos and compare the indented concave contours of the far cheek before treatment compared to the smooth contour of the upper cheek area in the after photos. Now look at the frontal and lateral view. Notice in the frontal view after photo her face looks less harsh, softer and more balanced. The lateral view shows how harsh the mouth area is in its anterior projection which is then balanced with the anterior cheek augmented. The volume lost in the temple areas have also been replaced and contribute to her overall appearance being more refreshed and youthful.
