This 57 year old female presented with concerns for jowling, the appearance of her neck, and the loss of midface volume. During surgical consultation to address her goals, we identified volume deficiency of the temples and lower 2/3 of the face, left more than right, jowling, and moderate cervicomental laxity with platysmal diastasis and banding. To address her concerns, she underwent a lower facelift with platysmaplasty and autologous fat transfer from the medial thigh to the temples, lower eyelids and cheeks. At her three-month follow up visit, the patient had achieved a nice, smooth neck and jawline with good improvement in cervicomental laxity. She has also retained an adequate amount of volume from her fat transfer which has augmented her lower eyelid, cheek and temple regions well.
