This patient is a 61 year old actor who desires facial rejuvenation to restore his youthful appearance as well as improvement in the appearance of his nose.
The patient chose to undergo a transconjunctival lower eyelid blepharoplasty, full face autologous fat transfer, lower facelift with neck liposuction and a cosmetic rhinoplasty. The patient lacked donor cartilage from his septum due to a previous septoplasty and so therefore he had cadaveric rib cartilage utilized.
His presurgery frontal and lateral view demonstrates significant lower eyelid bags and volume loss of his cheeks and mandibular area. It also demonstrates the significant jowling and sagging of his neck skin. Examination of his nose from the frontal view shows a C shaped deformity to the right and a hanging tip which is confirmed on his lateral view. The oblique views confirm the above findings. He also has a depression in the rim of the nose at the junction of the tip and sidewall which is most evident on oblique view.
His postoperative photographs demonstrate an amazing improvement in the appearance of his lower eyelids and the interface between the lower eyelids and cheeks, the nose, the jawline and neck. The frontal view shows the general overall improvement in facial aging as the patient now has a rested appearance but pay specific attention to the significant improvement in the lower eyelid bags, the improvement in the jawline from both the lower facelift and adding autologous fat to the pre-jowls sulcus. The lateral view also confirms the improvement in the lower eyelid contour as well as augmentation of the cheeks anteriorly. The heavy bags under the eyes are gone and the entire face has a fuller and younger appearance.
The straightening of the nose is significant as well. The lateral view shows a slight elevation of the nasal tip and improved dorsal contour as well as improvement in the tip/alar junction on lateral view. His jawline is much smoother without the protruding jowls. The lateral view shows off the new neck line that has been brought back significantly removing any appearance of “turkey neck”. The oblique view illustrates that the deep lines around the mouth have diminished and close-up view of the eyes also demonstrates the findings of diminished lines.
