
This 59-year-old patient consulted with Dr. Hudson Frey, a double-board-certified facial plastic surgeon, about her aesthetic concerns. She expressed concern over her "jowls, wrinkles, and droopy eyelids that fold over." She had a history of using non-surgical aesthetic treatments, including hyaluronic acid filler and neuromodulators, as well as skin resurfacing treatments like microneedling. She stated she desired a more comprehensive and longer-lasting intervention.
On physical examination, Dr. Frey found that while her brow height was adequate, she had upper eyelid dermatochalasis with deep-set eyes. Her lower eyelids had mild nasal compartment steatoblepharon but more prominent lower eyelid skin laxity and rhytids. He noted good snap-back and lower eyelid tone. She had generalized facial volume deficiency, including in the perioral region, the orbital groove, and the malar groove. Jowling and cervical laxity with platysmal banding were prominent. Dr. Frey customized a surgical plan to include a deep-plane lower facelift with platysmaplasty, an upper blepharoplasty, a lower eyelid skin pinch, and fat transfer.
Upon review of the patient's 3-month post-operative photos, her results are remarkable. Her natural, youthful appearance is highlighted by her defined jawline and contoured neck. Her lower face has no sign of jowling and her midface has beautiful, youthful proportions due to the tissues being repositioned. The small details of the skin pinch and fat transfer further enhance this patient's natural beauty.
