Rhinoplasty
This 57 year-old male patient presented in clinic desiring cosmetic improvement for the prominent appearance of his ears as well as the improvement for the external asymmetry of his nose; a result of a college football injury. For improvement of these areas an Otoplasty and a Septorhinoplasty were performed (although the patient denied breathing issues, examination indicated trauma to septum was apparent).
His preoperative assessment of his nose showed significant deviation and asymmetry of both the tip and dorsum. His lateral view shows collapse of the nasal dorsum with a pseudo-hump due to the deficiency. His ears are very prominent with stiff difficult to work with cartilage.
At the time of surgery the pre-operative findings were confirmed although the septum was much more severely deviated than thought and required significant repair. Sutures and onlay grafts were used to repair the nasal tip asymmetry and a dorsal onlay graft was used to repair the deficient dorsum in two locations.
His before and after frontal views show remarkable improvement in the appearance of his protruding ears. There is a slight asymmetry of the right from the left, which is not bothersome and could be further adjusted, but as a result of the stiff cartilage. Regarding the nose the frontal view shows the remarkable improvement in the asymmetry of the nose with excellent refinement.
The lateral view of the ears shows a natural placement of his ear fold; a result of rotating the ears using a suturing technique. Regarding the nose, the view shows significant improvement in the dorsal contour as correction of the nasal tip droop.
